Doula vs Midwife: What Is the Difference?

A doula provides non-clinical emotional, physical, and educational support. A midwife is a licensed healthcare provider who manages pregnancy, labor, and birth clinically. The two roles are different, and they often work together on the same care team to give families both clinical care and continuous support.
What does a midwife do?
A midwife is a trained, licensed healthcare provider who provides clinical care during pregnancy, labor, birth, and the postpartum period. Midwives perform exams, monitor the health of the parent and baby, manage labor clinically, and attend births. Their scope and licensing vary by state.
What does a doula do?
A doula provides continuous emotional, physical, and educational support. Doulas do not provide clinical care, perform exams, or manage labor medically. A doula's focus is comfort, communication, advocacy, and presence, always working alongside the licensed providers on the family's care team.
How doulas and midwives work together
Many families choose both: a midwife or physician for clinical care and a doula for continuous support. The doula helps the family communicate with their provider, use comfort measures, and feel supported, while the provider manages the clinical side of care.
Which one do you need?
Clinical care during pregnancy and birth requires a licensed provider, such as a physician or midwife. A doula is an addition, not a replacement. If you are considering doula support, ask your provider how a doula can fit into your care plan.
Does doula training make you a midwife?
No. Doula certification is not a healthcare license and does not qualify anyone to provide clinical care. Becoming a midwife requires separate, licensed clinical training. Black Pearl's certification program is doula training, not clinical training.
